Anyone have a formation in their custom playbook go haywire?

I put in flip snugs formation and it won't even show up in games. The formation name is there, but it shows up as a bunch of 5 wide plays.

Tried re-editing and going back and adding the plays to no avail.

Do I need to create an entirely new playbook?
