This is a work around. If you're on the Time Out screen just hit pause and make your subs there.

They will work immediately and you will still be able to make coaching adjustments on the pause screen when you resume.

For some reason, the TO menu treats subs as an in-game sub and not a TO sub which is why it waits until the next dead ball to insert the players.
