Pitcher hurt during warmups??

It's the bottom of the 8th inning and I'm at home, ahead by 1 run. I pinch hit for my 3B with a lefty 1B who proceeds to ground out. The next half inning starts with a cut to my 1B at 3B before my closer starts his warmup tosses. The last warmup pitch is wildly outside despite being soft tossed down the middle. The next batter is announced and I go to make a defensive substitution to get my backup SS in the game to play 3B. As soon as he's substituted in, the screen cuts immediately to my bullpen with the message "replace the injured pitcher". There was no notification of any injury and no actual pitches thrown in the game. After the game I got a message that he strained his bicep and will be out a few days.

Did he somehow get hurt on that final warmup pitch, did my substitution cause a glitch of some sort, or did something else happen? Has this happened to anyone else before??
