Franchise mode: only 2/3 X factor slots on offense and defense

I have 3+ X factor players on each side of the ball so I constantly swap out to pick my 3 . Suddenly it showing on my adjust lineup screen “2/3 zone abilities” for each and can only swap out guys , can’t figure out how to get back to having 3 active ... very confused please help thank you

Go into the depth chart menu (the "normal" one, not the one with the pics). Hit the reset button.

Go back into the visual depth chart and it should be fixed (if not, just back out and go back into the visual depth chart).

You might need to rinse/repeat a couple of times, but that takes care of it.
