Use the report button in the lower left. It's the icon that looks like a triangle with a vertical line in it below username/location.Steelers : IX, X, XIII, XIV, XL, XLIII

Thank you. If a user, or users, report a posting that clearly violated TOS, how do we know if any action is taken against them?Comment

You can flag posts and report them to the admins. However, there is no indication back to the person who reported the issue if any action is taken against the person who posted. It's a hole in the process.
I know that there are regular offenders who have many complaints against them throughout the years and postings reported, but they never get suspended or banned. Yet others who are relatively new users can get banned for life based on one post.
It does not appear to be an equitable process.Comment

If you see their name turn pink you know action happened. Beyond that, I don't think OS sends a letter to your parents asking for the offender to be grounded from the Internet.
I think the ''flag'' 13ulvl is referring to are flag infractions placed in the bottom of the offending post. This was done by moderators. A regular OS member could not place those ''flags'' there. I felt like they were helpful to new members so they could see what type behavior wasn't tolerated. You could then click on the yellow or red flag to read the notes the moderator made.
