To the OP, I had something very similar happen the second game I played. Ball was grounded to pitcher. No buttons were hit/tapped, nothing. Pitcher grabbed the ball and..... (I hit O, which means first base) nope, threw to shortstop...

WHAT!?! What base is shortstop??? At first I thought I was hitting L1 or R1 for the middle man coming in from the outfield. Nope, happened 2 more times total in other games as well.

I cannot reproduce the incident on demand, but I can honestly say it does happen. Very weird. It's not a game breaking issue, but it can mean the different of a win/loss in some cases.
But, all in all,.. yes, I can say the same happened to me. --you are not alone--

I doesn't fix your problem, but I assume the SS was covering second base?

i finally have my problem "fixed" - i made a mistake with the fielding settings - i forgot to change the "throwing decision" to off (to be found with r1 in the fielding settings) - ever since no problems occurred

hope this may help some other guys

What i noticed what i was doing was i never released the button in a timely matter after i pitched. The ball is in play and my finger is still on the X button for a slight second. So when i pick up the ball the fielder just threw it home. Had to teach my self to release after throwing fastballs. Weird thing is it only happens after i throw fastballs
