This unfortunately definitely costs me games. I never throw guys out at second because I am so afraid of overthrowing the first baseman on pick-offs.

To add to the confusion, it looks like they never updated the Loading Screen that still says to double tap for quick move.

I'm pretty sure the "Controls" menu is correct though. Single tap for quick move, double tap for casual move.

So the game has two different controls listed?

          Re: Wild pickoff throws

Well, I thought about maybe it has to do with the Pitcher's Arm Accuracy. So I decided to test it.

I got the Yankees and started with CC Sabathia (88 Arm Strength, 73 Accuracy). I intentionally walked the first batter and decided to do 10 pickoff throws. Not a single one of them was overthrown.

So I brought in Michael Pineda (77 Arm Strength, 29 Accuracy). I did 10 pickoff throws with him. Not a single one of them was overthrown.

I thought that was weird, so I figured I would try it with a different team. I picked the Astros and decided to start with Jarred Cosart (75 Arm Strength, 43 Accuracy). I made 2 pickoff throws. The first throw I held the button. The throw was right on the money. The second throw I just tapped the button and he overthrew first base.

So I brought in Chad Qualls (82 Arm Strength, 78 Accuracy). I made 2 pickoff throws with him. The first one I held the button, and he was very accurate with it. The 2nd one I tapped the button and he overthrew first base too.

So my conclusion is... I have absolutely no idea why it does it.
