A) I have starter stamina at 7, and I think lots of people do the same; it allows for a guy with average stamina (say 80) to go 100-110 pitches with relative ease, and allows the horses of the world (90+ stam) to go 110-120.
B) There's also the whole "max effort" thing (using max effort more often = fewer total pitches in a pitcher's stamina pile, so to speak), but I believe that's dependent on which pitching system you're using so I dunno. -

I have the stamina slider at 0 and I'm not having anything an issue. There are a combination of factors I have noticed that affect a pitchers energy during a game, and his management through a season. But every 5 days my starter is ready to go at 100 and rarely slightly less. I honestly think there are too many variables in play to determine how, why or when these pitchers performances are effected by their stamina rating and in game energy level.
My best advice to you take care with your pitchers management if that's something you're not doing. I don't fret when a guy is not near 100 either, I've used bullpen guys for one/two AB having barely half of their "energy" in tight situations. Not a common practice by me, but it can be done. If they are around 70 or above they are definitely pitching "available" to give me at least one inning by my standards.
