i understand that each face button represents a base, but sometimes you hit the according button and he throws it to the wrong base. what am i doing wrong?

for instance, a player was attempting to steal second before i released the pitch. i hit triangle and he still just threw the ball to homplate instead of second.

Base throwing controls changes if your playing RTTS mode.

yes i still had the ball in the pitchers hand.

As stated before RTTS controls are based on the fielding view, triangle becomes hom, square becomes first, etc.

That being said, I've had a very similar situation occur in franchise where I pressed right on the right stick and the ball was thrown to third, its happened twice in 10 games so far. I thought it was just something I did, a brain dead move but now I'm wondering.

You can change the RTTS controls so they match franchise controls, then you won't get confused.
