Hmm, trying to picture this in my head. Lineout to SS off pitcher's head, then each base runner gets thrown out for failing to tag up? Hard to imagine this occurring any other way.
And yes, I'm aware of the only reason a play like this can exist - Showdown. I wondered if it could happen any way other than intentionally running on an obvious fly out.
I only ever turned one, way back in 2010 (offline), seen below. Sharp grounder to third, Panda tags the passing runner, then goes around the horn. Pretty sure that MLB 10 The Show was also the title that had a trophy for a triple play, so that was fortunate.

I actually did it once this season in my custom franchise! First time I've ever even seen it happen in game. I record all my games, so I've got a recording of it somewhere. It was not an unassisted triple play, as I recall it was a 5 to 4 to 3 triple play.
I managed to get one in my season this year. The batter hit the ball close enough to third that my fielders momentum took him right to the bag, he threw it to second and then it went to first.
I also got one in RTTS. Similar play. It actually surprised me since the AI loves to drop perfect bunts with any player, my RTTS is a third baseman. I was playing on the grass but still managed to pull it off somehow.
