    Try a different controller. Could be a button activating on its own.

        It's a standing R2 dive tag while holding the ball, I can do it anytime on demand when there are runners on the bases. Could be R1 instead if you have one-button jump/dive on. The animation from a standing position is slightly different from the one while the player is running (he doesn't reach out quite as far), so that's why it looks like he's just falling.

        Given the way my fingers rest on the controller, and how sensitive the trigger buttons can be on the Dualsense, I'm always wary of doing this on accident. I'm not gonna try to convince you that that accident is what's happening here (could be a faulty controller too although two of them doing the same thing reduces that possibility), but that's definitely the command the game is accepting somehow.

        Here's both versions of the dive tag:

        If you're using the two-button jump/dive option you also have the utterly useless ability to jump in the air with the ball with R1.
