I worked on 2K6 - 2K8. We implemented 5 sizes for every body part. IIRC: Feet, calves, thighs, waist, butt, gut, chest, biceps, forearm and neck. And 4 levels of uniform tightness. Tight, normal, baggy, extra baggy.

I doubt they could ever get away with that given their agreements with MLB and MLBPA. Maybe for creted players though, but I doubt it would be as crazy as some people would want. that editor is pretty full/cluttered as it is.
