    OOTP for a 2 player league?

    Can you use OOTP for two people living in different countries to pick one team each and play the same league together?

    What happens when you have to play each other?

    Yes you ca take in two people from different countries. It's simple.

      And then you each share the same save file? Or does your game get played via online connection? How does that work?

        You can definitely do this. My brother and I are currently playing a franchise where he controls the Tigers and I have the Cubs sometime in the 1980s. We both have TeamViewer, which lets him remotely access my pc to see my game. Then, we each have a manager which you will need to activate control in order to make trades, set lineups, etc.

        We play live games on teamviewer. It works basically as a normal game would if two people were sitting next to each other and wanted to manage against each other. If you want to share the file, you can compress the folder containing all of your league files ( contained in the file path saved_games I believe in your OOTP game files. Upload this compressed folder to a google drive and voila, a portable file. Your friend can make changes to the file and repeat the above steps.
