Pitching substitution question, HR error, and Fatigue question

I am leading 2-0 in the top of the 8th with the Red Sox. Seattle has guys on first and second, nobody out. Beckett was pitching but I bring in Okajima to face the left handed swinging catcher Clement. The computer decides to sac bunt, which is fine.. and he successfully does it. So now the CPU has guys on 2nd and 3rd, one out. Beltre coming up. I have Piniero and Papelbon warming up. Piniero is ready so I go to substitute him in.. but wait, it wont let me.. I am like, WTF. I go to the substituion screen and I go to sub pitcher and it says, "you cannot sub out a pitcher who has not faced at least one batter" Well this is a bug obviously as Okajima did face one batter, but I know a SAC bunt does not constitute an AB. Unreal.. So there is nothing I can do except pitch to Beltre. Well, first pitch he hits a rocket and it was hard to tell if it was going to hit the monster or get out of here. Well it did hit the "monster" and 2 runs scored.. So i go to the replay to see the hit and what do I see? The ball actually hit the Coke bottle, which is way the hell up above the monster.. it hit the Coke bottle, bounced back into the field and Beltre ended up with a double. Are you kidding?

Those 2 things pissed me off....So I finish the game and end up winning in extra innings...

One other observation.. during my franchise, through the first 7 games I havent changed any position players except for 2... I just recently had an off day and a 3 game series against Seattl coming up... well after the off day, my guys fatigue levels remained the same.. around 85-90%.. That was a Monday. I played Tuesdays game and then when I went to Wednesdays game, everyone's fatigue level went up to 100%... Obviously a bug... Apparently the day off rest time is a day late??

Anyone experience any of these bugs?

Regarding no.1, correct, a sac bunt doesn't constitute an AB. But there's quite a few things you could of done.
You could of pitched way out of the strikezone and then do a mound visit or sub in a pitcher then (i think the game lets you do this). But the correct strategic play would be walk Beltre, load the bases and then either:
    a) bring your infield in, sub your pitcher (you would of walked Beltre with your starting pitcher so this is perfectly legal) and pitch for a ground ball, throw home, that's one out. Then the next play would be the double-play.
    b) play the double-play, and give up 1 run.
Most often you come out giving up 1 run instead of 2 or more.
Best scenario you come out giving up no runs (1 out at plate + the double-play). Also you could legally use your bullpen in these situations.

    No.2:
Yeah I did see that only once in 70+ games so far. Not sure if it's a bug or simply an attributes boost. My team was still tired on their day off, and by the time the 2nd day of the homestand came around, most of the team had close to 100% energy. This only happened once.

I can't remember who hit the HR but the ball went just left of the foul pole (in left field) and it was counted as a HR. I watched the replay many times and it didn't wrap around the pole or anything... that kinda P'd me off. But these things happen.
