Okay, so how exactly do you field a flyball?

Exactly what I do and I rarely have an issue. I had this happen once last night and I pretty much knew it was coming because there was something wrong with the way I approached the ball. The ball should have been caught, but the fact it wasn't was totally my fault.

There's many times I "let the CPU do the work" and my guy just stands there and never raises his hands and the ball lands right next to him and travels to the wall.

I've always pre-load and rarely have problems but the time I do it seems like a routine fly that I move over and just stand there and don't touch anything and he misses

Same here, I have tried preloading throws, holding left trigger and the fielding by the outfielders still stinks. It feels too clunky and very unfinished to me. Please fix this because I get to about the 4th inning in a close game and wind up getting ticked and turning the game off because of this.
