Here is the response I received...
The bug exists in the PS2 version, and is fixed for the XBox and PC version. But the bug is VERY infrequent (which is why we didn't fix it before PS2 was out the door, we hadn't seen it!! And I personally had played over 100 games on PS2, but anyway...)
In order for it to happen:
- There must be a hit-by-pitch. This causes a flag to be improperly set.
- Following the hit-by-pitch, there must NOT be any of the following events: Home Run, Ground Rule Double, Walk, or Certain kinds of foul balls. ANY of these will properly reset the flag and the bug will not occur unless another HBP occurs. Since many of these events are fairly common, this eliminates the problem in many cases.
- There must then be a man on 3rd with 2 outs. This happens every few innings on average.
- There must then be a fly ball to the outfield that is high enough such that the man can score from third before the ball is caught. This seems to happen on about 50% of the fly balls in the game; also, the man coming home is slowed slightly on the last few steps across home.

Any of those cannot occur but they reset the flag?? How do they reset the flag if they don't occur?? I don't get it.

No what he is saying for the bug to happen is any of the following can not occur, if they do occur it resets the flag properly and you want to the 2out man on 3rd problem.
