I was playing on XBL vs the White Sox, he brought in Willie Harris to pinch hit - grounded out to third. Struck out the next guy, the next guy grounded. Then the next guy struck out. No runners ever reached base. Anyone else seen this?

Yes, I've seen this happen once in the 30+ games I've played online. It happened in the first inning. No player substitutions or anything. My opponent simply needed to get the first "four" batters out to end the inning.
Another weird thing that I saw only once: I bunted foul (WAY foul, and not a third strike) and my baserunner proceeded to reach first safely
