You can, but you have to go in a manually edit to your preference. Same goes for franchise mode as well, but you can't edit players until they sign with a team.

So if for example you setup a MLB league and edit all the players you could still get a female signing as a free agent ?

Yes. You'll have to edit them after they sign.
