2k mirrors real life in that real life coaches tend to only call timeouts in response to runs or momentum swings. This is because the real NBA has regular television timeouts at set intervals which coaches utilize as "extra" timeouts to make standard adjustments. The timeout rule says each quarter must have a minimum of 2 timeouts. So if neither team has taken a TO by I think the 7 minute mark, the next dead ball triggers a mandatory TO. A coach will not call a TO at the 8 or 9 minute mark unless there is a huge momentum swing, and 2k has included this behavior, they just leave out the reason for the behavior.
The absence of TV timeouts has a under the radar yet still significant impact on gameplay.
If 2k makes coaches call more timeouts to mirror real life frequency of breaks in the action, the AI would burn all its timeouts before crunch time.

I think the larger issue, especially regarding player stamina, is the lack of fouls called. I play full 12 minute quarters and I see the average of 1 TO per team per quarter (which lines up with what NBA teams usually do). Then in the 4th if it’s close, I see teams use more of their timeouts.
But I’m only seeing 10-14 FTs per team per game, no matter how aggressive either my or the CPU team is. So because of that and no TV timeouts, there aren’t enough breaks in the action for subs to come in.Comment

hmmm My opponents almost always use all of their TOs and I had to reduce the fouls a little Lillard had 19 FTs my last game. Typically about 25 FTS per team in my 12 minute QtrsComment
