Why doesn't ID 03 alert you during/after games about injuries?

Any idea? Do I not have a feature turned on? This a great game, but I would like to know my injury status as it happens.

What do you mean? When you're actually playing the game, or when you're simming the games? For your players only or even other teams? As far as I know, ID 2003 has the best injury reporting in any game, because it has an actual sideline commentator that'll report on the injury: what the person has and how long it might take for the person to recover or how serious the injury is. And this is during the game that the injury took place. However, if you're looking for continued status on the injury you have to go to news, then injuries and look for the player in question to see what his status is.

I play every game because I am still learning ID in preparation for 04'. I have never been alerted to an injury. It is only when I go look why a certain player hasn't been subbed back in that I see the I next to their name. Any idea, or is this just a freakish thing that will balance itself out?

I've never noticed it happening that way. At leat, not if the injury's severe enough. If a player gets injured on the court, you'll see him either limping or holding his knee on one end of the court while the game goes on until it's a dead ball situation or whatever. Then the player gets subbed out and the commentators will refer to Akemi Takei for the scoop on what's going on with the player. She reports what happened on the sideline, then reports again after a while with more information about what type of injury it is and stuff. If you've never had this happen to you, I would think that some feature was disabled or something like that.

do you play with the volume on? Cuz the game doesn't show any visual injury descriptions during gametime, akemi takey or however you spell it just describes the injuries
