How long has this been a rule?

Bobcats/Cavs game. Charlotte has the ball down one, about 10 seconds left. Lebron pokes it away, several players scramble after it, eventually Lebron dives and replays clearly show he was the last to touch the ball as it goes out of bounds.

First the official correctly rules Bobcats ball. Then they confer on the sidelines and reverse that to Cavs ball after Lebron complains. Then the Bobcats coach has some words with the refs, who reconvene.

So eventually it becomes a jump ball? Is that what you do when the refs don't know what happened?

I think instant replay can only be used for last second shots and fights.
