This is something that plagues virtually every NBA coach.
A perfect example is what I saw today.
The Lakers and Heat are tied with 44 secs left. The Heat just called a time-out and instead of running a play that would take about 10-12 secs of the clock and basically guarenteeing themselves another position (most lilkely the last of the game) they choose to use up the full 24 secs. I'm seen this scenario play out time and time and it bugs the hell out of me why coaches don't go for the 2 for 1 when you still have at least 1 time-out left and have 10-14 secs to run a play (will even be better prepared if you're coming out of a time-out).
I understand if you do this, there's a chance the other team will jack up a shot right away or take a shot and grab an offensive rebound, but this rarely ever happens.
The 2 for 1 is the most "undervalued" strategy in the NBA.
