My HB went out with a twisted ankle for 1 week during week 8 and is still out in week 12. Why does it say 1 week and yet it has been 4?

It is like in the NFL where you don't really know how long they will be out. Some come back early but most take longer than they expect. Try going to Auto Substitution once you have started a game and changing the injury setting. I think it is on Probable you can change it to Questionable or doubtful. This might bring the RB back but he will not be as good as before but he might still be better than your backup. -
