Check the cable connection, make sure there aren't any bent pins on the connectors and it's firmly plugged in.
If you have access to a 2nd monitor, try using it for a night or two to see if it does the same thing. If it does, I'd assume vid card. Open the case and make sure the vid card is firmly seated. If it is, you might need a new vid card.
If it does not give you that pattern with the 2nd monitor, I'd assume the monitor is going bad. -

Could be a number of things. What type of monitor are you using? What type of graphics card are you using? Have you added anything to the PC lately, speakers, anything? When you turn your PC off for awhile, and turn it back on does it instantly do it?Comment

That is really weird. I rarely have problems with my computer so i can't exactly help with that. Good luck fixing it though
