You have to donate 20 bucks.

Yes to get a custom avatar you can either donate $20 or purchase an OS T-shirt. The donation helps to pay the server costs, bandwidth and ISP fees to host the site.

The custom avatar is OS's way of showing appreciation for the donation.

The description changes based on number of posts.

To have a personalized description, yes it's rolled into the same $20 donation or T-shirt purchase.

If I recall correctly, the last non-donator to be a mod was PDiddyPop, and he still had to use stock avatars and titles. So no, (I assume) you don't have to pay to be a mod, but that doesn't get you a custom title and avatar.

You don't donate for the avatar and title anyway; you donate because you like the site and want to help the owners keep it running in its current state and help it to improve. They do this all out of their own pocket and don't make any sort of monetary profit from it.

I thought the donations were to help fund Steve's nuclear program.
