Cold. I like to keep my mouth cool and fresh, warm would feel weird.

Drink? Hell no, swish and spit. As far as warm I don't think it kills any additional germs unless you use really hot.

Cool water gives your mouth a better feeling anyway and I use Listerine so I don't need warm water to kill germs.

I've always been told that warm or hot tap water is more likely to contain bacteria in the first place since the water potentially sits still in the water heater for hours and bacteria can develop in there. That's why you should never drink warm tap water. It's what I've been told anyway.

Cold. All the way. Warm feels funky and wrong.

Swish and spit, followed by the Listerine.

Mostly warm water but it doesn't really matter. It's not like I won't brush if I must use cold water.

And out of respect of people that I visit I always use cold water. I don't want to use their hot/warm water.

I live in an apartment so I don't pay for water. Not an issue for me but I realize it's something others pay for every month.

So I paid attention and when I am not in the shower brushing my teeth, I use cold water. I only use hot/warm when I brush my teeth in the shower.
