Application form layout design help in Microsoft Word

You know how on forms there might be something underneath the lines you need to write on in small print, to tell you what to write on the line? For instance with this form:

Look where it says things like "Last," "First," "Street," and pretty much anything else. Do you know how to make this effect in Microsoft Word? I've tried maybe superscripting the words, but then the line underneath will push the underscore I created for the next line down, or if I try and underline it the superscripted word will be underlined, as opposed to the baseline itself.

Anyone know any tricks to put those words underneath the baselines in small print like that?

I guess you could possibly try superscripting the words into the line before?

Sure but when I do, how can I create a line underneath the superscript at the baseline level (not directly beneath the superscripted word)? I can't quite crack that part.

Why not drawing the lines and then just create text-boxes under each line? Like this (you have an arrow pointing to show you the text-box tool):

You can assign the text-box no fill color/line color and you're good to go. Hope that's what you meant. But I agree, Word is terrible and so not flexible. I would NEVER ever use Word for that. Thank God for InDesign (and other graphics programs).

Agreed. My suggestion for a solution was going to be InDesign. I honestly wouldn't even know where to start with this in Word, but maybe you might have an easier time with this task if you tried Publisher?

If you have to use Word, do the form by creating tables, and then make the borders of the tables with no fill. It will allow you to customize the size of each field in the table along with the font size/style as well.
