I think we are best suited sticking to the posting guidelines suggested for this thread rather than throwing theories out here and there. Stick to showing proof of it happening and the exact settings you used when it happened. That is the only way to debug the glitch.

Especially since I have multiple devs checking out this thread specifically for information and the pot shots are not appreciated.

No adjustments or audibles. Coaching adjustments are all on default.

This is on All Madden with custom sliders. (Can go find and add the sliders if needed. )
