I don't know what "HM kicker" means. I also don't watch the NFL so I don't know if Janikowski like is "extremely good" or "extremely bad" (but since you're setting the sliders to 0, I'm assuming it means "extremely good.")
I don't play coach mode, but for every FGA I switch teams and let the CPU kick for me so, for FGs, it's like I play coach mode. I do this because if I kicked myself my kickers would always be 100% accurate.
My CPU sliders are 40 FGP and 35 FGA.
This has worked very well for me as my guys miss FGs at a pretty realistic rate. (Which is what's great about it since you need to recruit kickers with good KPW AND KAC.
Here are the career numbers for the kickers that I've had during my 25 year dynasty.

the average FG% for the top 100 kickers in the country is 74.2%. My kickers (who are always upper echelon kickers) have made 78.9% during their careers (and again they would've made roughly 98% had I controlled the kickers myself). My guys have only made 41% of kicks beyond 50 yds and have even missed some from inside 30 yds.
In short, using 40/35 FGP/FGA with a CPU controlled kicker has worked very, very well for me.

I just recently started a coach mode dynasty and now I see that it's nothing like letting the CPU kick for me. I don't understand why it's different, but I see the same problem that the OP saw with my kicker never missing.
So, now I'm just doing the same thing that I did when I played the games myself. After I fail a 3rd down attempt and I know that I'm gonna kick a FG I'll switch teams and let the CPU kick for me. I haven't kicked enough FGs like this in my coach mode dynasty to know if it works as well as I posted earlier, but I expect it to.
If you're gonna do this there are 2 important notes:
- Makes sure that you swtich teams before the play calling screen comes up, otherwise the switching won't take place until the next play.
- Switch to your opponent, don't go CPU v CPU. This is because sometimes (pretty often if your kicker has a weak leg) the CPU controlling your team will decide to go for it instead of kicking a FG. If this happens I'll make the defense jump offsides and switch teams back to my own. Then I'll chew the play clock (the game clock won't be running because of the previous penalty) and take a delay of game to put the ball back in its original spot. Then I'll kick the FG myself.
