Both DB's on the right side of the field will cover the receiver that runs a middle/seam route, leaving the outside WR wide open. I've also seen my DB inexplicably start running away from a deep receiver for no real reason, so I definitely have seen the behavior the OP is referring to. I play mostly on All-American, if that matters.

That one happens to me all of the time, or they both just run into each other somehow as well
