Because if the CF's chasing down a ball over his head to the wall, he should be throwing to a relay man. Not directly to home plate.

So in this case, the outfielder's throw decision isn't fitting the situation either.
It definitely did happen. I was trying to keep my shutout going, but I was mentally screaming "throw the ball, throw the ball...why are you slowly tossing the ball back in...." I don't recall whether I pressed the second or home plate button, but an "urgency" animation definitely did not trigger at all. Whether it was to homeplate or second base, "contextual urgency" animation should have triggered.

Edit: let's say it's an outfielder (i.e. Josh Reddick or Puig, etc.) with a strong arm, they're likely going to want to gun down the runner at home plate without going through the cut off man.

I also play on basically default sliders which don't alter the fielder's arm strength or reaction, YMMV if you lower them - I'd imagine plays can turn out quite differently when a fielder gets to the ball later or can't throw as hard.
I also play on default sliders. The only thing that i bump up to 100% is the pitch speed.
