    So why wouldn't injured players in A ball be called up when healthy and MLB caliber?

      They will...but not until sometime is hurt on the MLB roster, most likely at a similar position.

        Yes this is a known issue. What you have to do is create a separate "experienced" profile for the the teams you want on auto. I put the teams I want to control on "Experienced" and then create a separate "Experienced 2" profile with everything on auto and save it and then change the file associations.
