Two outs, bases loaded in the 9th, leading by one. Get a routine ground ball to my second baseman and I tell him to throw to first, then put down my controller to have a celebratory sip of my beer after getting out of a jam. Only to see my second baseman bobble the ball and have it fall in front of him, still with plenty of time to pick it up and throw him out at first, except the command gets canceled by the error and I've put the controller down like an idiot. I drop my drink in a panic and try and pick up the controller in time to make the out, I obviously don't do it in time so the tying run comes in. The next man up predictable gets the walk off hit, meanwhile my carpet is covered in broken glass and beer.

It's a new personal best for MLB the Show stupidity for me

I've had the occasional wrong base throw due to RTTS, but the biggest mistake ive made would be tossing Big Papi a changeup that stayed over the plate for too long on an 0-2 count. That home run cost me the game

Just had a heart breaker earlier ago and what made it worse was I had a PERFECT Game right at my finger tips. I was the Orioles playing the Rangers with Dylan Bundy the starting pitcher. No hits and no runs allowed through 8 innings.... then the top of the 9th happened.

Up 2-0 and Bundy gave up his first hit all game in the top of the 9th. With the Perfect Game no more, I pulled him for my closer. Next thing I know the Rangers pull off a 4 run inning in the top of the 9th after Bundy exited and the Rangers won 4-2 after I couldn't pull off the comeback in the bottom of the 9th.

Bases loaded, 1 out, up 3-0 in the bottom of the 7th. Groundball up the middle, SS fields it, steps on second, doesn't throw to first. Ended up giving up 6 runs in that inning and losing when I sent Posey home from second in the bottom of the 9th to try to tie it up. Note to self, don't send Posey to try and score on Jay Bruce.
