9-3 putouts
Anyone noticed that 9-3 putouts are occasionally missed? In about 90 franchise games, I've probably turned 3 or 4. I could have easily turned more if the 1B player decided to stand on the bag. Seriously, so many times I've had a great throwing arm in RF and some slow slugger jogging down the line. The throw would have easily beat him but 1B isn't standing on the bag.

Its to keep things realistic. 4 or 5 years ago they would always cover and it was pretty common to get a 9-3 putout. You having 3 in 90 games is already extremely unrealistic. From 2000 to 2013 (about 30,000 games) only 16 were turned. Then there were 3 in 2014 including Jose Bautista turning 2 in back to back games. As mentioned by others, it was a quick-fix patch a few games ago in order to prevent people abusing 9-3 putouts.
And it was left in (and not fixed properly instead) because ... reasons, I guess.
It looks silly each time a ball is hit hard enough to try it, and you should have an out or at least a close play if the 1B was standing on the bag.Comment

I play with button accuracy and even when the 1b covers first the game intentionally turns your accuracy to yellow so you pull the 1b off the bag even though I obviously stopped the meter in the green.

I try it every time but it only works if there was a runner on first and the 1B holds him on. Definitely should happen more though based on game physics.Comment

If your defense is in a shift, and the ball is lined to the rightfielder, you can definitely get an occasional 9-3 put out to snuff a rally.
I have done it maybe three times this year, but it seems only if the 1B is in his position for the Extreme Left Shift.
It makes sense, as it must fool the logic, with where the throw could come from being played by the 2nd baseman in right field.

However, how many times has there been a 9-3 put out this year?
How many 9-3 putouts have their been in the last 10 years?

On the flip side if people were getting thrown out at first by the right fielder they would be complaining about that and saying base hits to right field are broken.
There is nothing “broken” about it. How often do you see a first baseman cover first on a single to right field?
The OP has shown that it can be done but not routinely. Stats from real life baseball tell you that it’s an extremely rare occurrence just like in the Show.
As for “rocket” bunts that is a discussion for another thread.

My perspective is that the first baseman not covering the bag isn’t a “fix to an element that is broken”, but rather simulating what you see in actual baseball.

How many first basemen do you see run to cover the bag on balls hit to right field? It's not "broken". It would be "broken" if runners WERE being thrown out at first other than every millennium.
