In 2K10 and 2K11 the fatigue model was lousy imo. Did they fix the fact the DH position still got tired over the course of a season? A DH is considered a rest position. Some teams don't like to have a full time DH just so they can rotate their players through it to give them a break. I don't want it to be a full day's rest but a 1/2 day would suffice. This ticked me off in the past.
Also, 2K11 wasn't as bad as 2K10 when using a pinch runner or hitter but as soon as you subbed a guy in the game he automatically got some fatigue, even if it was just to go up and strike out. So instead of getting a 5%-10% boost in energy for resting the guy he loses all that and gets another 1% off his energy just for one pinch hit AB. Makes no sense. Give them a 1% hit for the AB but subtract that from the rest energy. Don't just hit the reset button and nullify 8 innings of rest with a 9th inning AB. Made playing in the NL very annoying.
Also, short season fatigue was even worse. They just drop games off the menu but don't compensate for the extra time off so your players never get tired and you don't have to manage a pitching staff at all.
Fatigue is one area where 2K got it wrong in the past imo. I can't say if these were fixed in 2K12 since I didn't play that version.
