Great episode, but I agree that it may have confirmed the theory I'm fond of, which is the whole "group therapy" deal.
Think of a giant mental hospital. We don't know how the "survivors" made it there, but it would explain the flashbacks (each individuals' past fears and memories). Last night Hurley was in a trance, giving his fears and thoughts to a psychologist, and thus his flashbacks. Next week will be the next survivor's session, which will have to build off the "island" events that were visualized by Hurley, and so on. The patients who experience true breakthroughs "die", and are removed from the experiment.
It's either that or the whole Heaven/Hell/Purgatory/Limbo theory, which I also feel got some weight last night with "Henry Gale" telling Locke that even "God doesn't know how long we've been here...no more then the rest of the world". It kind of implies a "Limbo" experience seperate from God, and when they've made their peace the person dies and goes to their ultimate fate.

I can't believe no one has mentioned the photograph that the Dr. took. It was during the day, people were around when he took the picture of Hurley & 'Dave". Well when the Dr. showed Hurley the picture, there was no one else around and it showed it was night time through the window.

Bear with me. It appears that the "survivors" are all in a Matrix-like world. They are all hooked up to the same machine and the island world is like the world of all of their thoughts/imaginations. Many of the Lost message boards talk about the "collective conscience." Well, everyone is injecting their thoughts/imaginations/experiences into the island world. For example, Walt has read a comic book about Polar Bears, then, all of the sudden, a Polar Bear shows up. Hurley's numbers that he heard in the asylum appear everywhere on the island.
Furthermore, what people have wished for in their real lives have come true in the island world. Locke can now magically walk, Sun is miraculously pregnant (even though Jin is sterile), Hurley has a hot chick sweating him, Jack was able to see his father again after his father died in the real world, Walt and Micheal bonded, Ecko was able to come to terms with his brother's death, etc.
As for people dieing (Boone, Shannon, lady in ocean, Arst) - if the mind dies, the body soon therafter follows. Just like in the Matrix, if you die in the Matrix, you die in real life.
This theory also explains a few other things - why the survivors survived a plane crash; the "security system" which is really the survivors' past deeds coming back to haunt them, etc.
How the survivors came to be hooked up to the machines is anyone's guess, but it seems like some evil scientific experiment.
If this theory has been stated by someone else on a Lost message board, I truly did not attempt to copy it and was completely unaware of it - I have no time to read all those threads. This idea just came to me last night after Dave and Hurley's talk.
