                The only people that know are Diggle, Felicity, Sarah, Roy, the occasional villain, and eventually Slade.

                And I can't even remember 100% if Roy knows.

                            Yeah at times it seems like there are more, but that's just because of how close he is to everyone else, and how often he interacts with them as Green Arrow.

                                It's probably not hard for high ranking government agents to find out who Arrow is if they really wanted to.
