Could be, but it looks like Hector isn't in the best health anyway. If Nacho seeks Mike's expertise will Gus's desire to delay his enemy's end play into this?
Finally caught up with the series. Didn't give it much hope after the pilot, but glad I stuck with it.

As for the pill, I can't see it as the way Nacho gets his revenge on Hector.
I feel like that would be lazy and very sloppy for the writers to use the same type of "weapon" they used in BB.
I can see him taking away the pills, which will result in some attack if Hector gets too riled up.
Hector scrambling for pills that aren't there wouldn't be very interesting. Breaking Bad used poison a few times (off the top of my head I count three), but I would not want to see that in this case.
If I had to guess Mike will do the dirty work, and it will be more violent than poison.
I can see Nacho replacing his pills with sugar pills so when he does have another attack nothing happens. Wasn't he in a wheelchair in Breaking Bad because he had a stroke?

Maybe in the southwest US, but he still answers to the cartel. Even during Hector's incapacitation his nephews were seemingly representing him, but certainly Gus's power grew when the uncle got sick.
My reaction as Jimmy was blubbering in the office at the end of this episode:
