The best way to learn is to have a friend either at home or online who knows what they're doing practice with you.

RT and left analog or right? And what direction would that be on they are trying to stand up? What direction if they are trying to take your back?

Right Trigger + Right Stick in direction they attempt to go.

Skip to 2:20 in this clip https://www.twitch.tv/phobiaos/v/54957890

What you will notice is I deny 4 transitions back to back before I attempt to move. Twice you see him try to move RIGHT, so that would be hold RT then flick right stick to the right right after he attempts to move. Then he tries to go left twice, as you see him grab my wrist. I then deny just like the last time BUT instead deny by RT + RS Left.

The ground game and denying transitions will come down to learning the subtle cues which show you what direction they are attempting to pass. Some are easy to read and others are very difficult. I'd suggest the same thing Aholbert suggest,

So as a newbie to the game, are you doing R2 + Right Stick left/right AND pressing triangle or square? Or do you stop the transition attempt first and then strike?

First to be clear, you have 4 directions you need to be prepared to defend.

R2 + RS Up
R2 + RS Down
R2 + RS Left
R2 + RS Right

Next, you need to realize you don't hold R2 + RS a direction "attempting" to defend where you think they are transition. You must flick the RS in the direction they attempt to move AFTER they attempt move. That is a critical thing, because the window is small and holding before they attempt the move will just result in them moving freely.

Lastly, you can strike when ever you wish obviously, but you can't deny or attempt a transition AND strike at the same time! If you deny a transition then throw a strike, be prepared to attempt another deny right after the strike. The only way you can prevent an open pass while striking is to rise up in a postured position (RS UP). At that point you can rain down strikes until they pull your head down or block your incoming strikes, after they do either of those they can attempt a pass.

Once you get down the methodical nature of the ground game it starts to form a rhythm. Stay calm, look to defend transition attempts, land strikes when the opening presents itself, but whatever you do.....don't spam transition attempts panicking. Higher level people will quickly put you away if you panic.

how are you even getting your denials off? I can't get them even when I know the transition is coming. I can do them in practice mode no problems.
