Denying *successful* TD's into double underhooks?

I was fighting against a Hendo that was able to end up in double underhooks over and over and over after I filled the entire TD meter. He denied a few attempts into underhooks (I don't know how to do this, but it looked legit). Then I caught him off guard with low stam and my TD meter easily filled all the way up. Before we hit the ground, animation blipped around looking glitchy, and we end up standing and he's got double underhooks. This happened at least 5 or 6 times. Impossible to take down.

How does one pull this off legitimately? How do you know if your fighter has this move or the TD denial sub? (it's bizarre there are moves with no explanation. You can't even find how to do these moves by Googling).

Is there a known bug with this move?

L2 + R2 up on the RIGHT stick. Backwards for takedowncounter and down for sub counter.

I've encountered this again. Guy did this to me no less than 10 times in a fight. My meter filled up ALL the way over and over, and still, this:

This dude was also repeatedly able to transition out of submissive positions before my meter would even start filling up, even though I was holding my transition preemptively.
