My guess as well just from a few minutes of trailers.
For anyone wondering if they'd be interested in the plot of this movie, consider that the novel was written by the same man who wrote "Mystic River" and "Gone, Baby, Gone."
Now, you may not have enjoyed this films, but I thought they were fantastic and both have interesting plot twists. So, yes, expect that this film will have a similar theme.

Wow I'm really surprised this only got 65% on RT. It looks great, but maybe I'm just a huge Scorsese fan. I loved Gone Baby Gone so if it is like that I'll be happy.NBA: New York Knicks

Saw it last night and loved it. I loved the music and Leonardo Dicaprio was great. He is becoming one of my favorite actors. The movie was nicely done and I came into it with a ton of expectations and was not letdown at all. I know I will buy this day 1. Saw the latest showing last night, and it captured everything that I wanted to see. Not much replayability in my eyes other than one more showing to look back and go "Oooh, now I get it," but it was very elemental and every scene counted for a build-up towards what was one of the greater drama endings since Fight Club, Se7en, and Memento.
For me, Scorsese is like Tim Burton: not that he directs like him, but when he finds a proper niche in the screenplay which he's directing for, it's something that comes out the way that I best see that it should come out. For instance, I don't see anyone else doing Corpse Bride except for Burton. That doesn't necessarily mean that I like the directing style, but it's more because I'm not as fascinated in the movie's style. Much the same, I could have seen others do The Departed, and frankly as much as I love that movie, I almost think that I would have preferred to mention one or two to have directed that. This isn't harking Scorsese's work at all, I'm just saying that I think I found where I like him most in Shutter Island. The thing that struck out most were the moments of complete silence. That is, even when drops of rainwater falls, the sound is drowned out for the ensuing intensity that Leo's character feels in his mind. It works out so purposefully and really helps contrast what happens afterward.
This is a mind-mangling film from beginning to end on the part of choosing whose mind you want to enter and whose thoughts you want to focus on. For people that think they'll be lost or confused in the movie's words, there is no need to worry, you will be completely fine. Characters are well spelled out, and their purposes served become clear as day. There are moments of vagueness that become moments of clarity, and what the eyes see and the ears hear, the mind believes.
Does anyone know if the movie follows pretty closely to the novel? My wife and I went to see this today...Good stuff and highly recommend. We enjoyed it. I realize that Leonardo is getting a lot of praise in this movie (rightfully so), but I have to say that Ben Kingsley is the man!!STEELERS INDIANS CELTICS

I saw this tonight - awesome film. Leo was great, as usual, but I thought everybody else did a wonderful job as well. Which leads me to this...
SpoilerMaybe I'm alone in this analysis, but until the end I thought the primary characters were, not acting poorly per se, but something with their performances just wasn't clicking. "Chuck" seemed to be trying too hard - his character seemed too on the nose to be truly believable. Every scene with him (until the reveal) made me think of the black-in-white 40s films. Not in the sense that it was believable, but in the sense that the performance was too flat and cardboard cutout. I think his "role-playing" was the most obvious since he was essentially the only faking his role, but even the way the Doctor would reveal some the information seemed cheesy and corny - "Its like she just...evaporated...through the walls."
Like I said, maybe I'm alone in my assessment but that's what really cemented the twists at the end for me and I don't see how anybody could not think he wasn't crazy.
I loved all the Hitchcock vibes throughout, too.
