If you're saying "Damn, the #Braves are looking really good tonight!" you don't need to hashtag "Braves" because you're already putting it in your tweet.
BUT if you say "Heyward 2-run shot! #Braves", you're putting it in there so that anyone who searches "Braves" will see your tweet even though it's not part of your main message.

I think there's exceptions to the "the hashtag must exist" rule.
Like, if I'm watching a random wrestling event, let's say the Royal Rumble from 2006 (as I was yesterday), I might throw a #RoyalRumble2006 tag on to the end of my tweets so people know what the hell I'm talking about.

Definitely. Hashtags should only be used when you want to include something you tweet in a search string. Also, a hashtag has to EXIST. Hashtagging things like #BeefJerkyBlues is stupid because nobody else is tweeting about that so it's pointless.
