Im pretty sure there is only 1 injury slider (which I believe is broken but thats a different thread) but anyway its in the my madden menu under game settings

The main menu under the My Madden tab, go to options and its there where you change skill level, speed threshold, etc...

And the injury slider only affects played games. It does not have an effect on simmed games and the injuries that occur during them.
