i did it by accident as well. I had broken loose for about a 20 yard gain and accidentally hit the pitch button and the ball fly in the air about 10 yards backwards.

I thought L1 is the pitch button. It almost seems like it's happening randomly when I'm just sprinting down the field?!

Maybe a button is sticking a little bit, try a different controller.
