Custom playbook glitched/bugged?

Anyone else having the issue with custom play books where a formation 'goes missing'? While flipping through my formations a random one will suddenly be blank and the player icons all consolidate to one player dot and are located out of the formation graphic... This then throws off the names of each following formation (the names of formations don't match up with the personnel).
What I've tried to fix it:
Deleting the formation that was bugged from playbook (resulted in another formation becoming bugged)
Saving the playbook with different name.
Adding and re adding all plays/formations

Is anyone else experiencing this? Or does anyone have an idea for a workaround?

All plays appear to be there. Just after the bugged formations the name doesn't match formation... Throws me off a bit... Not the end of the world though..

I had one where I had no kick recovery plays besides the base return middle. No onside kick, no nothing.

This happens to me with every custom playbook i make.
