Slider from 38-50 and yes, they seem to come in bunches. I have played games without any though.

I have seen one in two games post patch, Fumble slider at 50, Tkl at 45, AP.

Yes. I've had 3 through 9 weeks. And I see at least 1 per week throughout the league.
