I noticed that myself, but I didn't think it happened everytime. Maybe I was wrong, but I don't think it's a big deal anyways.

This is not a bug. It was designed this way to keep pitch counts more realistic. Already been discussed.

I really hate the HR Camera this year. It completely breaks my immersion. I'm really hoping that the option to turn it off is just really well hidden.

I can't figure out how to change the Home Run Camera either. Anybody got any ideas?

I just played a Quick Pitch count game and LOVED it. But there was 1 thing. My SP i took out in 8th inning with 2 outs. Brought in RP. It regenerated new pitch Count but at 3-2 but said my RP had - pitches. SO 5 pitches were either not counted or given to my SP
