OP:

I take it you're using button throwing (press the button and the length of the press determines throw strength). That throwing mode is the most ratings-dependent of any of the modes - the only thing you can control is the velocity of the throw. You might be able to affect the accuracy indirectly by how hard you throw it, but you don't have direct control over that. It's similar to directional hitting in this regard - ratings are just much more impactful.

The other modes - analog and button accuracy - give you a greater degree of user control over the accuracy. You might want to consider familiarizing yourself with one of them if you're having too many throws go offline.

If you do insist on using regular button throwing, you might want to consider lowering the throwing error slider. Just be aware this lowers accuracy for the CPU as well, and you'll be seeing less of their throws going offline too. Other than that, as others have said be aware of your fielders' throwing ratings, because if they're bad it's going to cause more offline throws no matter which mode you're using.

I've noticed in this year's game, a lot of the time my catcher can't handle it even if it is an accurate throw. Don't know if that's Bruce Maxwell isn't as good defensively as Yadier Molina or if it's something new.

This is the same issue I have. I can get an accurate throw to the place a decent amount of the time, but usually the catcher (anyone I use) either doesn't field the ball cleanly, or takes FOREVER to apply the tag. If I throw anyone out at home, it's usually because they made a bad decision, and are out by 3 to 4 steps.
