I left fatigue at 50 and don't change the substitutions. I either manually sub the HB or do formation subs. If I run the HB 3-4 times in a row, his icon changes color. Or if I have multiple catches...

Yes the players do fatigue and get injured. Most of my injuries have just been for the rest of the game, but I finally had one where my HB was out 2 games.
