I get what you're saying now, my bad. I think coach mode is a playable game mode, like play now or the mascot game. I'm not sure about how the Coaching Carousel works with Coach Mode, maybe someone...

I would guess by not picking a team when you play a game that would cause the game to be shown with the Broadcast Cam. I don't play coach mode though so I'm not 100% sure

Broadcast cam is not a playable camera angle; it is only available for use in coach mode. I believe the answer to your first question is yes also
